At Well Life Outreach, we connect Nevada’s elderly, underserved, and uninsured residents with the care and support they need to live healthier, more independent lives. Community Case Management Hub

Our case managers guide seniors and families through every step of their healthcare journey—from hospital discharge to home recovery—connecting them with the right providers, benefits, and community resources to reduce readmissions and improve quality of life.

Durable Medical Equipment (DME) Program

We collect, sanitize, and distribute donated wheelchairs, walkers, canes, and other essential medical equipment to individuals who cannot afford them—restoring independence and mobility with compassion.

Through workshops and community programs, we empower seniors, caregivers, and families with practical knowledge on navigating healthcare, managing chronic conditions, and preventing hospital readmissions.

Nevada Medicaid Provider for Targeted Case Management

  • Adults who are non-seriously mentally ill (non-SMI) with a mental illness
  • Adults who are seriously mentally ill (SMI)
  • Children and adolescents who are severely emotionally disturbed (SED)
  • Children and adolescents who are non-severely emotionally disturbed (non-SED) with a mental illness
  • Individuals with Intellectual Disabilities (IID) and related conditions

We focus on these target groups to gain access to medical, social, educational, and other support services.
