Well Life Outreach CDC Inc. is an approved Nevada Medicaid provider offering Targeted Case Management (TCM) services. Our program helps individuals connect with healthcare providers, community resources, and support services that promote stability and overall well-being.
Our case managers work closely with clients, families, healthcare providers, and community partners to coordinate services and ensure individuals receive the care they need.
Who May Qualify
Targeted Case Management services may be available to individuals eligible for Nevada Medicaid, including:
- Adults who are non-seriously mentally ill (non-SMI) with a mental illness
- Adults who are seriously mentally ill (SMI)
- Children and adolescents who are severely emotionally disturbed (SED)
- Children and adolescents who are non-severely emotionally disturbed (non-SED) with a mental illness
- Individuals with Intellectual Disabilities (IID) and related conditions
