We provide comprehensive case management services that enhance knowledge, promote collaboration, and support holistic well-being for the elderly, uninsured, and underserved communities across the state.
Well Life Outreach is also a Nevada Medicaid provider delivering Targeted Case Management services to connect individuals with essential healthcare and community resources.

With over 30 years of nursing experience, including more than a decade in case management, Sharon San Gaspar brings both skill and compassion to her leadership at Well Life Outreach. She shares,

Outside of work, Sharon loves spending time with her family, traveling to different countries, and enjoying good food — experiences that remind her of the value of connection and joy in every season of life.

Gerlie Comahig’s 14 years of nursing experience add compassion, empathy, and dedication to her role at Well Life Outreach. Her clinical background and commitment to patient-centered care align deeply with WLO’s mission to ensure that every individual—regardless of circumstance—has access to health, housing, and essential resources.

In her spare time, Gerlie enjoys relaxing and staying connected with the people she loves—watching TV, making calls to friends and family, cooking homemade meals, and sharing a good glass of wine filled with laughter. These simple moments of connection help her stay balanced, grounded, and grateful—values she carries into her work every day.

Mindy embodies the compassion and integrity that define Well Life Outreach. Her expertise in care coordination and patient advocacy allows her to connect clinical excellence with genuine empathy — ensuring that each person she serves receives not just care, but understanding and respect.

In her spare time, Mindy finds renewal through travel, hiking, and spending quality moments with her family. These experiences allow her to reflect, stay grounded, and bring the same sense of balance and gratitude into her work and service to others.


Secretary / Care Coordination Assistant
With over 18 years of experience in the medical field and an academic background in Business Administration, Shawna Ornelas blends professional expertise with heartfelt service. Her role as a Care Coordination Assistant reflects her belief in patient empowerment and education — helping families navigate complex healthcare systems with confidence and compassion.
“I believe in Well Life’s mission because I believe in our team. Together, we will address social determinants of health and give patients the tools they need to navigate healthcare systems. Having been in the medical field for over 18 years, I truly believe that with the right tools and education, we can bridge the gap.”
Shawna enjoys arts and crafts with her daughters, refurbishing furniture, and taking road trips with her family. Her creativity and love for family life bring warmth and perspective to her work in healthcare and community service.
Board Member
With 18 years of experience as a Licensed Master Social Worker and Accredited Case Manager, Renee Cisco brings deep expertise in navigating the social factors that impact health outcomes. Her work has centered on serving individuals affected by social determinants of health (SDOH), ensuring that vulnerable populations receive equitable access to care and support.
“Being a clinical social worker has allowed me to serve those impacted by SDOH who are at increased risk for worsening health outcomes. We need stronger community-based relationships to advocate for their well-being — just as Well Life Outreach strives to achieve.”
Renee enjoys taking road trips with her husband, discovering new restaurants, and sipping her favorite Boba drinks — without the boba. Her compassionate heart and joyful spirit reflect her belief that community connection is the foundation of healing.


Board Member
A hospital-based internal medicine physician with more than 14 years of experience, Dr. Brian Chang has served patients across Nevada and California. A dedicated educator, he also mentors medical residents and advocates for integrated, team-based approaches to care. His commitment to Well Life Outreach reflects his belief that coordinated, compassionate healthcare can transform lives and strengthen entire communities.
